"DALLAS (CBSDFW.COM) - Both directions of the LBJ Freeway were shut down early Tuesday afternoon due to a dump truck crash between Webb Chapel Road and Marsh Lane. The incident happened at about 10:30 a.m. when the truck appeared to have jumped over the center concrete median.
Chopper 11 was live over the scene and spotted at least two other vehicles involved in the crash, one on each side of the highway. Both of those cars were pinned underneath each end of the dump truck.
According to Jason Evans with Dallas Fire-Rescue, there has been one confirmed death as a result of this incident. Another individual had to be transported away from the scene after being extricated from their car, but did not have life threatening injuries. Authorities have not yet said what may have caused the dump truck to jump over the median and into oncoming traffic.
The area where the crash took place is part of the LBJ Express Project, where road construction has already been impacting the regular flow of traffic heading east and west through north Dallas."

Princeton Rabbi Dead, 2 Seriously Injured in Crash Outside of Riverside Elementary School.

"A 74-year-old Princeton Rabbi was killed and two people seriously injured after a BMW careened down Riverside Drive on Thursday morning without braking, crossed to the other side of the road and struck two parked cars.
Rabbi James Diamond of Princeton, who was getting into a parked Toyota Prius at the time of the crash, was pronounced dead at the scene of the accident, which occurred around 9:40 a.m.
Diamond is the former director of Princeton University's Center of Jewish Life, and moved to Princeton from St. Louis. According to a 1995 article posted on the Princeton University website, he is married with three children.
The driver of the BMW, Eric Maltz, 20, of Princeton, was taken to Capital Health Regional Medical Center. Rabbi Robert Freedman, 63, of Princeton, who was in the Prius was also taken to the trauma center. Both of their conditions are classified as serious but not life-threatening, according to Princeton Police.
Freedman is a former cantor at the Jewish Center of Princeton. Both Freedman and Diamond were reportedly leaving a weekly Talmud study group when the accident occurred.
Maltz was driving a 2003 green BMW sedan was driving south down Riverside (away from Route 27) when it crossed into the southbound lane and struck a parked Toyota Camry, pushing it into a parked Toyota Prius parked behind it, police said.
The BMW pushed both cars about 500 feet, and the Prius ended up even further away, across the street and onto private property.
"There are two obvious things," Princeton Police Capt. Sutter said at the scene Thursday morning. "The BMW was traveling at a high rate of speed and didn't appear to attempt to brake."
There was no immediate indication that Maltz suffered any type of medical condition that may have led to the crash, Police Sgt. Mike Cifelli said.
The speed limit on the street, which is in a school zone, is 25 mph.
The crash remains under investigation by Princeton Police, with the assistance of the Mercer County Prosecutor's Office. "

"Ky. 52 east of Danville was temporarily shut down Tuesday afternoon after a woman crashed her car into a utility pole.

Boyle County Sheriff’s Office responded to the scene to discover the woman had lost control of the car while going around a curve.

Police said the wet highway played a factor in the accident.

The woman was not injured and refused transportation to the hospital.

Police said the utility pole was damaged, and the highway was closed while cleanup took place.
The crash is being investigated by Deputy Taylor Bottom."

"A Bayonne man who may have been racing with another driver crashed his car into a wrought-iron fence early yesterday morning, sending all five people in the car to the hospital, authorities said.
Ralph Bacani, 19, of Avenue E in Bayonne, was issued numerous motor vehicle violations, including summonses for reckless driving and violating the conditions of his special learner’s permit, Jersey City police spokesman Stan Eason said.
The four passengers in the car had to be extricated by Jersey City firefighters after the 4 a.m. crash on Kennedy Boulevard near Stuyvesant Avenue, reports said.
All five people were treated at the Jersey City Medical Center for injuries that were not life threatening.
Eason said that “speeding and/or racing” may have played a role in the crash, which is still under investigation.
Bacani slammed into a parked car and a parking meter before the car came to rest on a wrought-iron fence next to a wheelchair access ramp in front of a doctor’s office."

Man Rushed To Surgery In Tulsa Rollover Wreck.

"TULSA, Oklahoma -

A man was seriously injured after his car rolled on U.S. Highway 169 at about 5 a.m. Saturday. It happened just south of the 81st Street exit in the southbound lanes of the highway.

Tulsa police say the man, who was about 30 years old, was trying to pass another vehicle when he lost control of his SUV. The SUV rolled several times, trapping the driver inside. He was freed by Tulsa firefighters.
Carl Bennett, the driver of the car being passed, said he saw the SUV speeding by him then saw the underside of the car right in his front windshield.
"It was all pretty fast," he said. "He was rolling level with my steering wheel, so I cut over to the right to get out of it, and that's where he stopped."
Traffic was diverted from the highway for a couple of hours.
"It does appear that alcohol was involved tonight," said Corporal R.W. Solomon, Tulsa Police.
Officers say the driver was in very serious condition and was rushed to surgery Saturday morning.
"As of right now, he's still with us," Solomon said."

"(RAINIER, Ore.) - One person is confirmed deceased Tuesday morning following a two-vehicle fatal traffic crash on Highway 30 about three miles east of Rainier. Oregon State Police (OSP) troopers report Highway 30 is completely blocked and may be for several hours.

A suggested detour for northbound and southbound traffic that needs to travel through this area on Highway 30 is to use the Longview/Kelso bridge and Interstate 5 routes. An alternative route to get to or from the Oregon coast is to use Highway 26.

According to OSP Master Sergeant Larry Lucas, preliminary information indicates on November 30, 2010 at approximately 6:00 a.m. a sport utility vehicle driven by an adult male was northbound on Highway 30 near milepost 43 when it drifted onto the shoulder.

The driver over-corrected to the left into the southbound lane where it collided head-on with a large truck. The sport utility vehicle's driver was ejected and pronounced deceased at the scene. The truck's driver was not injured.

No more information will be released until later today following the on-scene investigation and attempts to notify next of kin.

OSP troopers are being assisted at the scene by ODOT and Columbia River Fire & Rescue."

Burglar Drives SUV Over Retaining Wall and Into a House.

"(HILLSBORO, Ore.) - On Saturday Washington County Deputies responded to a Burglary call in the 8400 block of Canyon Drive. Items taken from the burglary included a flat screen television. While deputies were investigating the burglary, another call of a motor vehicle crash came in to dispatch.

Deputies responded to the nearby crash and upon arrival located a 1998 Toyota 4-Runner perched on its front end and leaning against a house. Deputies contacted two men walking from the crash scene, 26-year-old Desmond Chand of Beaverton and 26-years-old Simon Saleem of Los Angles. They learned that Desmond Chand was the driver of the 4-Runner and Simon Saleem was the passenger.

When deputies looked inside the vehicle, they located a flat screen TV taken from the burglary scene. As a Deputy spoke to Mr. Chand and Mr. Saleem, Mr. Chand ran from the scene. Mr. Saleem was taken into custody. Washington County Deputies setup containment and conducted a canine track, but did not locate Desmond Chand.

After interviewing Mr. Saleem, deputies learn that Mr. Chand was drinking heavily prior to driving. Mr. Saleem was also injured during the crash; he was treated at the scene. Simon Saleem was arrested for Burglary I, Conspiracy to Commit Theft I, and lodged in the Washington County Jail.

Desmond Chand is wanted for the crimes of Burglary I, Theft I, Fail to Perform Duties of a Driver – Felony, Assault IV, Reckless Endangering, Criminal Mischief II x2, DUII, Attempt to Elude – Misdemeanor, and Reckless Driving.

Anyone with information on Mr. Chands whereabouts is asked to contact the Washington County Sheriff’s Office via dispatch at 503-629-0111."

"A Tukwila man was seriously injured in a crash that embedded a guardrail inside the passenger compartment of the car he was driving.
Haybe Ahmed was driving a Toyota Camry northbound on Highway 509 in Burien at 12:11 p.m. Tuesday when he drove across a lane and struck a guardrail separating northbound and southbound traffic near Southeast 128th Street, according to the State Patrol.
Ahmed, 46, was trapped in the car until medics extracted him. He was listed in serious condition at Harborview Medical Center in Seattle on Tuesday evening.
Police investigating the crash haven't determined whether drugs or alcohol played a role.
The State Patrol says the guardrail prevented Ahmed's vehicle from crossing into oncoming traffic"

"NEW YORK (WABC) -- A school bus collided with a car and ended up slamming into a storefront in Brooklyn early Thursday.

The incident happened around 6:10 a.m. at the intersection of East 21st Street and Church Avenue in Flatbush.

Authorities say the mini school bus was hit from behind by a white four-door Toyota in the middle of the intersection after that driver ran a red ligth. Both vehicles went out of control, sending the bus careening into the building. The car crashed into a parked car.

"SPOKANE VALLEY, Wash.—Authorities said a teenage driver fell asleep at the wheel early Friday before crashing his truck.
Washington State Patrol troopers said happened just before 7 a.m. near Indiana and Pines in Spokane Valley.
Troopers said a 17-year-old driver fell asleep while crossing Highway 27 and went off the roadway to the right.  From there, the boy struck a concrete island, slammed through a street sign, and came to a rest on railroad tracks nearby.
The driver suffered minor injuries and refused any medical treatment at the scene.  A 16-year-old male passenger was not hurt.  Both boys were wearing their seatbelts.
Troopers cited the teenager with 2nd Degree Negligent Driving."

"A Portland man who was seriously injured in a fiery crash two miles west of Philomath on Saturday was reportedly in stable condition on Sunday afternoon, but faces charges related to the crash.
Biet Nguyen, 69, of Portland was cited with third-degree assault, driving under the influence of intoxicants and reckless driving after his pickup rolled and then slid on its roof down Highway 20. An oncoming van struck it and caught fire.
Driving east on Highway 20 toward Philomath on Saturday evening, Nan Perigo was the first witness on the scene at 4:44 p.m. She slowed when she saw a cloud of dust in front of her.
“The next thing I knew,” she said, “there was an upside-down car in front of me and a van on fire across the way.”
The driver of the van, Stephen Sever, 63, of Corvallis, seemed dazed by the impact, Perigo said. He hadn’t realized that his 1999 Ford Econoline had caught fire. Perigo and another bystander alerted him.
“We both hollered for him to get out,” she said. “… He was slow to react but once he realized what we were saying, he got out.”
He escaped out of the passenger side window. Emergency responders transported him to Good Samaritan Regional Medical Center, where he was treated for minor injuries.
Nguyen, upside down in a 2003 Toyota Tacoma pickup, was unconscious after the crash, Perigo said. She hadn’t noticed at first, but he was the driver who had passed her just minutes earlier.
Multiple people had called 911 prior to the crash, reporting that the pickup was weaving and might be driven by an intoxicated driver, according to Benton County sheriff deputies.
Investigators believe that Nguyen’s vehicle drifted off the highway and when it hit gravel, he jerked the wheel to get back onto the road. It caused the vehicle to flip and begin to slide on its roof.
Sever’s van crashed head-on into the front driver’s side of the upside-down pickup and came to rest in the ditch on the westbound side of the road.
After Sever escaped the burning van, Perigo and another bystander gathered fire extinguishers from the rows of cars that began to gather at the scene.
“We were concerned that the van was going to blow up and cause even more problems,” Perigo said. “We emptied four fire extinguishers on it and it was still going. It would slow it down but none of them would put it out.”
Philomath Fire Department put out the blaze and cut open the other vehicle to get to Nguyen.
“The roof was crushed,” Perigo said. “He (Nguyen) obviously rolled it hard.”
The crash resulted in the closure of Highway 20 for roughly three hours.
The Benton County Multi-Agency Crash Investigation Team assisted at the scene. Firefighters and EMTs from the Philomath Fire Department and the Corvallis Fire Department assisted, as well as the Oregon Department of Transportation.
The sheriff’s office is asking anyone who witnessed the crash or has any information to contact Sgt. David Peterson at 541-766-6820."

"A Toronto cop was seriously injured when he apparently lost control of his cruiser while racing to a crime scene and smashed into a car in East York this morning.
Witnesses say the officer, who may have been responding to a robbery, was southbound on Dawes Rd. near Victoria Park Ave. at a high rate of speed with lights flashing and sirens blaring when he crashed around 9:30 a.m.
Dinesh Rathasingam, 22, was driving home with friends after spending the night at a Tamil vigil downtown when the cruiser flew by his car.
He watched in his rearview mirror as the police vehicle slid sideways and slammed into a Toyota Corolla parked at the side of the road.
"I was almost in tears, I was so freaking out for his life," said Rathasingam, after he and his pals ran to the officer's aid and called 911.
One of the others in Rathasingam's car described it as "a massive hit."
"I'm still shaking," Gowrishangar Kamalanathan, 24, said.
He and others said the officer was out cold and bleeding badly from a head injury, but he seemed to regain consciousness before being whisked away by ambulance to the trauma centre at Sunnybrook hospital.
A 76-year-old man who was in the parked Corolla was taken to Sunnybrook hospital.
Toronto Police said the province's special investigations unit, which probes all incidents of serious injury or death involving police, was contacted and may invoke their mandate.

"“I think he’s dead, I think he’s dead.” Those were the words from one of the first witnesses on the scene of a fatal crash late Thursday night in Lorain. A car traveling east on East 42nd Street at a very high rate of speed first struck a truck then the curb at the corner of East 42nd and Seneca Ave before crashing into a tree.

Lorain Police, LifeCare Ambulance Paramedics and Lorain Fire Department all responded to the scene around 11:45pmm Thursday night but there was nothing that could be done for the driver who was trapped in the car. Approximately and hour later the driver was removed from the car and transported from the scene as Lorain Police continued with their investigation.

Harold Martin said he was sleeping at the time of the crash but his wife was sitting at the kitchen table playing cards. “She was just playing a game of solitaire when she heard the crash and she looked out and saw that it was right there in our yard.” Martin said that he has lived in that house for 13 years and although speeding and running stop signs is a constant problem in his neighborhood, nothing like this has ever happened before. “This is just something else; I guess it was only a matter of time.”"

Second Elderly Plantation Bay Woman Dies After May 3 Wreck With a Truck on US1.

"(censored), 76, a resident of Plantation Bay, died on May 4 from injuries sustained Thursday evening in a two-vehicle wreck just outside Plantation bay, where authorities said she lived. (censored), 68, who was riding with her, died on May 9 after battling critical injuries for four days.
The two women were involved in a violent crash between their Toyota and a pick-up truck hauling scrap metal. Look and Teske were airlifted on separate helicopters to Halifax Hospital in Daytona Beach. Paulo Rodrigues, the 36-year-old driver of the pick-up truck, a Palm Coast resident according to a Florida Highway Patrol reports, was in serious condition, and was transported by ground to Florida Hospital Flagler.
U.S. 1 in both directions was closed for three hours for about a mile on either side of Plantation Bay from the time of the crash, just before 5 p.m., to 8:22 p.m., by which time Roger’s Towing had cleared the scene.
The silver Toyota, according to preliminary findings by Florida Highway Patrol investigators at the scene, was stopped at the Stop sign on Plantation Bay and U.S.1. The Ford F-350 pick-up truck was traveling north on U.S. 1, in the outside lane. The Toyota attempted to pull out onto the southbound lanes of U.S. 1, crossing the northbound lanes and making a left-hand turn. The Toyota “violated the right-of-way of the F-350 Ford pick-up truck, at which point they made impact on U.S. 1.,” an FHP investigator said.
The truck’s scrap metal remained contained. But the violence of the impact embedded the truck’s front bender into the driver’s side of the Toyota, where it remained when wreckers separated the truck and the car. The car’s roof had been sheared off and its passenger side door cut and lifted to give rescuers access to the victims.  The Toyota’s steering wheel was cut off and laid by the side of the road."

N12 accident leaves five dead.

"An accident on the N12 between Glen Harvey and Fochville has left five people dead and another one person with critical injuries.

It is not clear as to how the accident happened, but it is believed that both vehicles may have been travelling in the direction of Randfontein from Potchefstroom when they collided.

When paramedics arrived, one of the vehicles was in the road and the other on the side of the the road. Both vehicles had been extensively damaged in the collision. The five occupants of the Toyota Corolla had suffered multiple and fatal injuries, and they were found entrapped in the wreckage. There was nothing further that could be done for them and they were declared deceased. A woman who was travelling in the vehicle is believed to have been pregnant, but this is as yet not confirmed.

The driver and only occupant of the bakkie was left with critical injuries and he was treated on scene before being taken to Krugersdorp Private Hospital by ambulance.

Police are still at the scene to conduct their necessary investigations into the cause of the fatal accident."

"A driver has been killed after a light motor vehicle and a articulated truck collided on the N2 near to the Kingsway off ramp this morning.
ER24 paramedics, who were on route to another medical emergency, came across the accidents which happen on the N2 south bound. The articulated truck that was carrying sugar cane was across the three lanes of the freeway and half way down the embankment. The light motor vehicle was found in the centre medium with extensive damage to the whole vehicle.
The driver of the light motor vehicle was found inside the car entrapped. ER24 paramedics assessed him and found that he had already succumbed to his injuries as he had suffered severe traumatic injuries. The driver of the truck was unharmed in the accident.
It is believed that the accident occurred on the N2 north bound and came across the centre medium and onto the oncoming traffic. The freeway was closed to motorist until the necessary authorities had investigated the scene.

"There were no injuries reported from a two-vehicle wreck that occurred on Turner McCall Boulevard near its intersection with East Ninth Street on Saturday at about 5:20 p.m.

Rome police and Rome-Floyd Fire Department officials responded to the wreck.

According to Pfc. Aaron Thacker:

Kara Winters, 26, of an Alabama Highway address, was driving her dark blue Toyota 4-Runner south on Turner McCall and tried to turn left onto East Ninth Street. She had stopped in the turning lane when Shane Woody, of a Calhoun Road address, was heading south in his older model Dodge Dakota truck, drifted into the turning lane and clipped her rear passenger’s side.

Woody’s truck traveled an estimated 30 yards before he ran head-on into a ditch.

Both Winters and Woody were unhurt from the wreck as well as Woody’s passenger, 20-year-old Melissa Hance.

Thacker said a small child was in the truck with Woody and Hance and was upset by the wreck, but didn’t appear to be injured. She was taken to Floyd Medical Center for a check-up.
