"HURRICANE CITY, Utah (ABC 4 News) - A Hildale/Colorado City Ambulance crashed into a Dodge Caravan and injured eleven on Saturday night.

According to Utah Highway Patrol, the ambulance was transporting a stroke victim, Ben Barlow, to Dixie Regional Hospital. EMT Lizzie Barlow of Colorado City was driving, and the patient's wife Martha Barlow was also in the ambulance, along with other EMTs and paramedics.

Officials say the minivan, driven by Christian Affolter of Switzerland, was traveling through the intersection of State Road 9 on a green light and began to turn left when the ambulance entered the intersection and t-boned the minivan.

Of the three passengers in the minivan, 8-year-old Serena Affolter sustained the majority of the impact and suffered extremely critical injuries. The other passengers, Daniela Affolter and 12-year-old Nicola Affolter, also suffered serious injuries. Serena was life flighted to University Medical Center in Las Vegas.

The passengers in the ambulance also sustained injuries, including a dislocated shoulder, officials say.

Ambulance driver Lizzie Barlow was issued a citation for traffic control signal violation."

Stolen car went airborne in fatal wreck.

"PHOENIX -- A suspected car thief is dead after driving car into some concrete barriers in the area of 31st Avenue and Encanto Boulevard early Thursday morning.
According to Sgt. Tommy Thompson of the Phoenix Police Department, the car was reported stolen from 4800 W. Encanto shortly before 4:30 a.m.
Both aerial and ground video from the scene show the mangled car, a 1986 Buick National, on its side.
The driver, an unidentified Hispanic male believed to be in his early 20s, was speeding east on Encanto Boulevard when he failed to negotiation the turn where the roadway bends on to southbound 31st Avenue.
"There's a concrete barrier there with some signage," explained Phoenix Police Lt. Chris Moore.
The car hit that concrete barrier went airborne, flying over a 7-foot chain-link fence before landing in a dirt lot. The Buick burst into flames on impact.
Bystanders ran to the car and put out the fire, but they were not able to save the driver.
Moore said the driver was not wearing a seat belt and was ejected from the car. That man died at the scene.
While it's not yet clear is alcohol or drugs were a factor in the deadly crash, Moore said speed is likely partially to blame."

Local woman hurt in 1-car crash.

"A Grand Haven Township woman suffered minor injuries when her car left the road and struck a tree late Tuesday afternoon.

Bonnie Thornell, 62, was driving her Cadillac west on Lincoln Street, west of 112^th Avenue in Robinson Township. Her car traveled across the eastbound lane, went down an embankment and into the tree, said Robinson Township Fire Chief Paul VanVelzen.

Thornell was wearing her seatbelt and the air bag deployed when the car crashed, VanVelzen said.

“She appeared to have minor injuries,” he noted. “She complained of chest pain. ... I thought it was going to be a lot worse."

Thornell was taken by ambulance to North Ottawa Community Hospital."

"YORKVILLE — A 28-year-old woman was rushed to the hospital with head trauma after being struck by a taxi that spun onto the sidewalk at the southeast corner of Second Avenue and East 79th Street Monday night, police and witnesses said.
The SUV taxi that hit the woman had first been involved in a crash with a Crown Victoria cab before spinning out of control, witnesses said.
The unidentified woman was taken to New York-Presbyterian/Weill Cornell Medical Center, FDNY officials said. She was stable early Tuesday, an NYPD spokesman said.
Delti Braha, 29, doorman at the Continental Palace, near the intersection, ran outside to help the woman just after hearing the loud boom about 7 p.m.
"She was face down, bleeding," said Braha, who was told by others on the block that she had been standing on the street when the cabs got into the accident.
"[The SUV] slammed into her and she was on the floor," he said. "It's sad."
An NYPD spokesman said there was no criminality was suspected "at this time," and that both drivers remained at the scene.
How the accident occurred was not immediately clear.
Police officers at the intersection said the SUV cab was making a left turn from Second Avenue onto East 79th Street.  One witness said she screamed when she saw the Crown Vic because it was speeding eastward on East 79th Street just before the incident happened.
But the driver of the Crown Vic, who declined to give his name, claimed that the other cab passed a red light while going south on Second Avenue.
"He was running the red light, so he hit the [pedestrian]," the Crown Vic driver, who appeared unharmed, claimed. "I'm really sorry for her friends, for her family, her children, whatever she has. It is stupid. All for $5. …I'm very sad for her. Honest, it's crazy."
Braha said it was not uncommon for cars to go fast at that intersection.
"Everybody speeds up," he said. "They're trying to catch the lights.""

"MIAMI COUNTY, Ohio —
Deputies in Miami County said one person suffered minor injuries when two vehicles collided Friday morning.
Investigators said two cars were traveling eastbound on Ohio 571 shortly before 9 a.m. when one of the vehicles stopped to make a left turn onto Ohio 55, according to News Center 7's Steve Baker. The second car collided with the vehicle that had stopped, and both vehicles flipped over, landing on their tops.
Two Ohio Department of Transportation crews working in the area of where the crash happened and ran to help those in the wreackage.
One driver was taken to a local hospital with minor injuries.
The crash happened about a mile outside the village of Laura."

"An accident at Fourth Street and Second Avenue North left quite the scene of diesel, oil, and antifreeze this afternoon.

Keith Seleen exited 394 and was turning onto Second Avenue at about 12:30 p.m. today when his Cadillac Escalade was struck by a Continental Carbonic Ice Products truck that ran a red light. Seleen said he never saw the truck coming.

"It's one of the tough intersections in town," Seleen said. "I'm just glad everyone is okay. I'm more concerned about everyone around that could have been injured."

Seleen said he's made it a habit to stop and look both ways before driving through intersections downtown, even when he has a green light.

Of course, one of the few times he doesn't check, Seleen gets into a car accident.

The Continental Carbonic driver declined to comment on the crash but did say he is okay and suffered no major injuries.

No injuries were reported, and the crash could've been much worse, Seleen said.

Jason Artemiuk was crossing Second Avenue with his wife and four-month-old baby when he heard the impact of the crash.

Artemiuk watched as Seleen's car spun and clipped the side of his car, which was parked on Second Avenue.

"I'm just glad we walked out later or the baby and I would have been hit," Artemiuk said.

Amy O'Brien, a witness, said this is the second accident she's seen in the area in the past two weeks. The previous one took place just a block away, at Fourth Street and First Avenue. She also said she witnessed an accident at the same intersection as today's accident many years ago."

Patient dies after ambulance is hit by SUV, overturns.

"GLENCOE, Ky. - A patient who was being taken to an area clinic for medical treatment died Tuesday after the ambulance he was riding in was struck by another vehicle and overturned in Northern Kentucky.
Two members of the Gallatin County Fire Department were transporting (censored), 59, of Dry Ridge, when an SUV struck the ambulance on U.S. 127 near Tapering Pointe Road in Glencoe just before 10 a.m.
Police say the SUV, driven by Hunter LeGrand, 22, of Warsaw, struck a pole and a mailbox before crossing the center line and colliding with the ambulance head-on.
The ambulance overturned and landed on its roof.
Little was flown to University Hospital by a medical helicopter where he died from his injuries. An EMT who was in the back of the ambulance with Little, David Waltz, 25, of Carrollton, was also flown to University Hospital by a medical helicopter.
The driver of the ambulance, Justin Shields, 21, of Ghent, was transported by another ambulance to University Hospital.
LeGrand had to be extricated from his vehicle before he was flown to University Hospital by a medical helicopter. Police believe he was under the influence of drugs when the accident occurred.
The conditions of the other victims have not been released.
Charges against LeGrand are pending.
The crash remains under investigation."

"ALBUQUERQUE (KRQE) - One person is dead and another is recovering at UNM Hospital after a head-on crash that shut down a major Albuquerque street Friday afternoon.
Bernalillo County sheriff's deputies reported (censored), 74, was driving north in his Hyundai Sonata when he clipped the rear of a Mustang in front of him on Broadway Boulevard SE at Murray Road.
Garcia then spun out of control and into oncoming traffic hitting a southbound Chevrolet Tahoe headed south.
Garcia died at the scene, and the other driver, whose name has not been released, suffered what deputies called severe injuries.
The driver of the Mustang was not injured.
The crash happened about 3 p.m., and at 8 p.m. the Tahoe driver was reported to be in surgery at UNMH.
Broadway south of Rio Bravo Boulevard was closed for about three hours.
Deputies said there was no sign of drug or alcohol use by Garcia although their investigation is not yet closed."

SE Portland woman killed in wrong way collision on I-84.

"NEAR CORBETT, Ore. - The Oregon State Police is investigating a fatal crash on Interstate 84 east of Corbett that involved a motorist driving the wrong way.

According to Lt. Gregg Hastings, a 2007 Kia Rio was traveling westbound in the eastbound lanes late Friday when it collided head on with a pickup driven by 53-year-old James Schneider of White Salmon, Wash.
The 36-year-old woman driving the Kia died at the scene. She was later identified as (censored) from Southeast Portland. She was wearing safety restraints and the car's airbags deployed, the Oregon State Police reported.
Schneider was taken to a Portland hospital with serious injuries.
Hastings said at least one driver complained about a wrong-way vehicle just before crash."

"A driver was injured Friday afternoon in crash on Wolf Hill Road in Melville.

The driver of a Honda Civic was traveling South on the road near Saxon Avenue when she lost control of her vehicle and slammed into a tree around noon.
The Melville Fire Department was on the scene for immediate assistance. The driver was then sent to Huntington Hospital with neck and back injuries, as well as lacerations to one leg, police said.
The driver's identity has not been released."

"PAWLEYS ISLAND, SC (WMBF) – The Highway 17 crash between a bus and a pickup truck has claimed the life of one person Tuesday night and left another in critical condition.

Bob Beebe, spokesman for Midway Fire Rescue, says a collision between a 1997 Dodge pickup truck and a Williamsburg County Transit bus on Highway 17 and Beaumont Road occurred just before 7 p.m. Tuesday and it claimed the life of the pickup truck driver, (censored) 47, of Pawleys Island, due to head trauma and left one bus passenger in critical condition. 
Officials say the bus driver, Joshua McCrea, of Kingstree, SC was traveling south when the truck crossed Highway17 and the bus struck the truck's driver's side.
Coroner Kenny Johnson tells WMBF News an autopsy is scheduled for Smith Wednesday morning at Grand Strand Regional Medical Center.
According to Beebe, there were 19 patients total and 12 were transported to the hospital. Three patients were taken to the Grand Strand Regional Hospital, two were taken to the Waccamaw Community Hospital and seven were taken to the Georgetown Memorial Hospital.
South Carolina Highway Patrol is investigating the incident, and Midway Fire Rescue officials say the bus had a surveillance camera onboard.  South Carolina Highway Patrol will reportedly review the footage to see who was a fault.
First responders from Murrells Inlet, Horry County and Georgetown City and County assisted the Midway Fire Rescue in their response.
Beebe confirmed, the southbound and northbound lanes have been opened and the accident scene has been cleared.
Alfonzo Allen, a friend to Smith, says he witnessed the crash, and he believes Smith never saw the bus coming.
"[Smith] was just easing out [onto the road] to go home, and I guess he didn't see the bus," Allen said. "The bus tried to miss him, and hit him."
Allen says he can't imagine life without his friend Smith in it.
"It's hard for me right because that's my best friend, somebody I look up to," Allen said. "That's my brother, my friend.""

"EL MONTE - All lanes of the westbound 10 Freeway were reopened late Tuesday morning at Peck Road, the site of an earlier fatal accident, according to the California Highway Patrol.
Three of the four lanes were closed for more than six hours Tuesday after a minivan driver was killed and two road construction workers were injured. The accident occured at 4:14 a.m. Tuesday, said the CHP.
Traffic lanes were reopened by around 10:30 a.m. Tuesday, according to the CHP, but not before creating a major traffic slowdown from El Monte to West Covina that may have added more than an hour to commute time for motorists driving to Los Angeles.
CHP Officer Ed Jacobs says a Ford Aerostar rear-ended a road work truck as the crew picked up traffic cones on westbound 10.

Jacobs says the minivan's driver died. Two road workers were taken to the hospital with minor to moderate injuries.

Jacobs says he can't confirm TV reports that a dog in the minivan also was killed.

The crash follows a Sunday accident that killed two road workers on Interstate 405 in Torrance. Authorities say two suspected drunken drivers collided, and one car slammed into repaving equipment."

"NEAR SUTHERLIN, Ore. -- A Roseburg woman is dead after an early morning crash near Sutherlin on Monday.
Oregon State Police say that at around 2:50 a.m., an unloaded log truck being driven by Kelly Kremer, 59, of Myrtle Creek, was heading westbound on Highway 138 West, about eight miles west of Sutherlin,
That's when officials say (censored), 28, of Roseburg, driving a Mazda 4-door sedan, crossed the center line and struck the log truck nearly head-on.
Troopers don't know at this point why she crossed the line, but the investigation is continuing.
Officials say Clift was not wearing a seat belt, and was driving with a suspended operator's license.
Kremer was transported by ambulance to Mercy Medical Center in Roseburg, where he was treated and released for minor injuries.
OSP officials say that he was using safety restraints.
Senior Trooper Robert Gorman from the Oregon State Police told KPIC News that there was alcohol found in Clift's vehicle, but do not know at this point if alcohol was a factor in the crash.

OSP troopers from the Roseburg Area Command office are continuing the investigation. Douglas County Sheriff's Office, Sutherlin Police Department and Kellogg Rural Fire District assisted at the scene.

The highway was closed for about four hours."

Man dies, crashes truck into Walmart Garden Center.

The Livingston Parish Coroner's Office was called to the scene Tuesday morning after a truck crashed into the Walmart in Watson, LA.

The crash happened around 9 a.m. Tuesday. Reports state the pickup truck left the roadway at Cane Market and Highway 16, crossed the parking lot and crashed into the Garden Center side of the store.

According to the Walker Police Department Facebook page, the driver of the pickup was extricated from the vehicle and was pronounced dead at the scene. Initial reports say the man suffered from a medical condition. It is not known if he died because of the crash, or because of a medical condition. His name has not been released.

No employees or customers of the Walmart in Watson, LA were injured.

The Louisiana State Police continue to investigate the crash."

2 teens, woman arrested after fatal 4-vehicle crash near airport.

"Two teenagers and a 32-year-old woman have been arrested in connection with a deadly crash in Lexington County.

The accident, involving four cars, happened Monday afternoon on Highway 302 near the Columbia Metropolitan Airport.

According to the highway patrol, 19-year-old Sheldon Dubois of Cayce was driving with a suspended license when he lost control of a black pickup truck and crashed into three other vehicles, including a U.S. Postal Service truck.

Dubois is facing several charges, including reckless homicide.

Lexington County Coroner Harry Harman said 64-year-old (censored) of Lexington was traveling in a white pickup truck when he was hit by Dubois. Mitchell was not wearing a seatbelt and was pronounced dead at the scene, the coroner said.

Two others were hospitalized.

Troopers say a 17-year-old passenger in Dubois' truck is also being charged with leaving the scene of crime.

According to highway patrol, 32-year-old Sharon Ellsworth, who was not at the scene of the accident, is also being charged for allowing Dubois to drive the vehicle.

Investigators are still trying to determine Ellsworth's relationship with the two teens."
