"Two men are dead after a morning crash in Augusta  County. 

According to state police, it happened around 7 a.m. along Woodrow Wilson Parkway near the ramp to Middlebrook Road.

A Chevrolet Equinox was traveling north on Route 262 when it crossed the centerline and struck a southbound GMC Sierra pickup truck.

The driver of the Equinox, James M. Bibb, 23, of Barboursville, died at the scene as did the driver of the Sierra, John D. Seale Jr., 56, of Millboro.

The highway was closed for a few hours while authorities investigated the scene and the crash was cleaned up."

Truck crashes into waiting room at Danville doctor's office.

"BOYLE COUNTY, Ky. (WKYT) - A close call for some patients at a Danville doctor's office after a truck slammed into a crowded waiting room.
It happened around 12:30 p.m. Tuesday afternoon at the Ephraim McDowell Heart and Vascular Institute on Ben Ali Drive.
A firefighter with the Danville fire department tells us A woman driving the truck crashed into a column in the front of the building, then put the pick-up in reverse and slammed into the waiting room.
Danville police identify the driver as 73-year-old Thelma Richardson of Junction City.
The fire department says she was taken to the hospital to be treated for non-life threatening injuries.
Officials with Ephraim McDowell say it might have been a good thing she hit the front of the building first because it got people away from the waiting room. "Well, yeah, it got everybody out of the way. If there hadn't been some means of distracting the people, then, yeah, there could have been some people seriously Hurt," said Keith Bridges with Ephraim McDowell Health
Two people inside the waiting room were treated for scratches and bruises.
The building was determined to be structurally safe and remains open for business.
Maintenance crews with Ephraim McDowell have already began repairing the damage."

Woman survives horrific crash in Danville.

"A Danville woman narrowly escaped serious injury Tuesday afternoon when a tractor-trailer rig loaded with toilet paper and paper towels overturned on top of her SUV as she was preparing to stop for a red light at the intersection of the Danville bypass and Lebanon Road.

“I can’t believe my head is not crushed,” Wendy Howard, 39, said at the scene as she looked at her GMC Envoy with the front of its cab caved in by the trailer, which was carrying a payload of about 45,000 pounds.

The crash occurred about 3 p.m., when the big rig, driven by Adolfo Penny of Clinton, Md., was southbound on the bypass and ran through the red light at Lebanon Road, witnesses told Danville police.

The big truck, owned by Crete Carrier Corp., collided with a Chevy pickup truck driven by Clifton Grubbs Jr. of Danville, who was traveling east through the intersection on Lebanon Road, and then careened into the northbound lanes. The tractor-trailer sideswiped a Ford pickup truck driven by Sabe Ruggles, 18, of Danville before the weight of the load shifted, causing the trailer to overturn on top of Howard’s vehicle, police said.

Grubbs was transported from the scene by ambulance to Ephraim McDowell Regional Medical Center for treatment of possible back and neck injuries, police said. No other injuries were reported.

The cab of the big truck ran through a guardrail on the right side of the northbound lanes before turning over on its side, leaving the trailer sprawled across three lanes of traffic. The intersection was finally cleared about 11 p.m., eight hours after the crash occurred, police said.

Because the trailer was damaged in the crash, it had to be partially unloaded before it could be brought upright, Danville Police Chief Tony Gray said.

The paper products, which had just been picked up at Wausau in Harrodsburg, were off-loaded onto pallets by hand and then reloaded on to another trailer using a forklift, Danville police Capt. James Monroe said.

Roberts Towing of Lexington brought two oversized wreckers to the scene in order to winch the big rig back on its wheels, Gray said. A tanker truck also was deployed to drain the diesel fuel from the overturned truck before it was set right, Monroe said.

Howard said she had been running errands and was on her way to pick up her daughter at Woodlawn Elementary School when the big truck flashed across her path as she was easing up to the stop light.

“It all happened so fast. The trailer just came in on me,” she said.

The top of the trailer smashed in the cab of Howard’s SUV, but she sustained no injuries and was able to unbuckle her seat belt and climb out through a back door, she said.

Several people at the scene speculated what might have happened to Howard if she had traveled another foot or two before the trailer toppled over on to her vehicle.

“It’s one of those things where you just go ‘Wow!’” said Danville Fire Chief Woody Ball. “Her guardian angel was flying with her, I suppose.”"

Crash Victim Dies After Van Wraps Around Tree.

"KOSCIUSKO COUNTY, Ind. (Indiana's NewsCenter) - Kosciusko County Sheriff's Department responded to an early morning fatal van crash.
Kosciusko County Deputy Coroner Michael Wilson pronounced Justin Ellis, 25, dead at the scene from blunt force trauma after he crashed into a tree south of State Road 25 in Kosciusko County.

Kosciusko County Sheriff’s Deputies and emergency personnel responded to a 911 call Saturday morning at approximately 1:59 a.m. of a van crashed into a tree south of State Road 25 on County Road 450 West. When officers arrived, they found a 1992 GMC van crushed and wrapped around a tree off the east side of the roadway.
Parkview Samaritan helicopter was called to the scene, but when paramedics arrived they pronounced the driver dead. Kosciusko County Deputy Coroner Michael Wilson pronounced Ellis dead at the scene from blunt force trauma.
Kosciusko County F.A.C.T./crash investigator’s stated that Ellis was driving southbound on County Road 450 West. Investigators said Ellis swerved off the road and crashed into a tree, but they are not sure why. The crash is still under investigation."

"PUTNAM COUNTY, Ind. (WISH) - The victims of a fatal accident on I-70 near the 43 mile marker Friday evening have been identified.
The Indiana State Police and Putnam County Coroner’s Office have identified the victims to be a mother and daughter.
Emma Bronson, 49, of Pascagoula, Mississippi, and her daughter Zacchia Bronson,3, were pronounced dead at the scene.
Mrs. Bronson succumbed chest injuries and her daughter succumbed head injuries, according to a release.
The driver of the BMW, Michael Bronson, 47, was treated at IU Health Methodist Hospital for neck and back pain. Bronson is the father and husband to the victims. He has been released from the hospital.
The driver of the tractor-trailer, Jordan Moore, is still hospitalized at IU Health Methodist Hospital with extensive injuries, police say.
The driver of the pickup, William Bertelmann, is still at Methodist undergoing observation.
A preliminary investigation Friday revealed that eastbound traffic was stopped for construction  when a tractor pulling a box trailer failed to stop and struck a pickup in the rear. The pickup then overturned off the roadway. The tractor-trailer continued heading forward and struck the rear of the BMW which then burst into flames.
The investigation is still ongoing.
No citations have been issued."

"A head on crash in Lorain Friday afternoon left one person dead at the scene trapped in a burning car.

Lorain Firefighters and LifeCare Paramedics responded to Henderson Bridge for a reported head on car crash. When Police and Firefighters arrived on scene they found a small car fully engulfed in flames with a person trapped inside. The car, driven by a 70-year-old male whose name is not being released yet, was seen driving north on Henderson Drive heading to the Bridge when it crossed the median and slammed head on into a red pick up truck. Witnesses at the scene say that the driver was slumped over the wheel when they saw him strike a guardrail and orange construction barrels before striking the pick up truck. The driver of the truck received minor injuries in the crash.

The Lorain County Coroner will do an autopsy to determine if the driver died of a medical condition prior to the crash or if the crash and fire killed him.

The Lorain Police Department immediately blocked all traffic to the Lofton Henderson Bridge and kept it closed for several hours while Traffic Investigators examined the scene."

Rollover on Route 2

"A man-driving west on Route 2, between Middle Ridge Road and Leavitt, late Saturday night went off the left side of the road and when he tried to get back in his lane he overcorrected and lost control. The truck he was driving spun and rolled over coming to a rest on its roof. The driver then fled the scene leaving the truck and hundreds of yards of debris. In the debris was the man’s Lorain County Metro Parks Ranger uniform, badge and gun belt, minus the gun. State Troopers, County Sheriff Deputies, Amherst Police Officers and Amherst Firefighters searched the area to find the gun.

The driver was later found at his home a short distance away with injuries to his arm and cuts on other parts of his body. LifeCare Ambulance Paramedics and Amherst Firefighters treated the man at his home while Ohio State Troopers and Lorain County Sheriff’s Deputies questioned the man. It was reported that the man had tried to claim that someone stole his pick up truck.

LifeCare took the man to Community Health Partners where he was later flown to Cleveland Metro Health Center."

Victims Identified in Fatal Crash North of Bismarck.

"Bismarck, ND -

Three people are dead and three injured in a morning crash north of Bismarck.
It happened around seven at the intersection of Washington and 1804.
The North Dakota Highway Patrol says the driver and two passengers in a van were killed and two other passengers injured.
The driver has been identified as 31-year-old Jose Manual Villareal-Ramirez of Mandan.  He was not wearing a seatbelt.  45-year-old Jose Pina-Moreno and 48-year-old Martin Meza-Bravo were also killed.  Two other passengers, 45-year-old Barry Zacher and 41-year-old Fabian Salgado-Monarrez were injured in the accident.  Monarrez was the only person wearing a seatbelt.
Authorities say the van ran the stop sign at Washington and was hit by a truck going east on 1804.
The driver of the truck, 54-year-old Bue William Thompson of Bismarck,  was the only person in the vehicle.
He was also injured.
All of the injured were transported to a local hospital.
North Dakota Highway Patrol Sgt.Dan Krueger says,"I don't believe that the weather was a factor, there was fog this morning and with visibility about half mile quarter mile visibility the vehicle was traveling north bound and disregarded the stop sign, vehicle 2 T-boned them sending them in to the ditch."

None of the five people in the van were wearing seat belts."

Dad, daughter killed by suspected drunk driver.

"A father and daughter were killed overnight in a head-on collision with a driver heading the wrong way on the eastbound QEW ramp to northbound Hwy 427.

A woman in the car, believed to be the mother of the dead teen, was taken to Sunnybrook Hospital with life-threatening injuries. Police say the family was out celebrating the woman’s 50th birthday.
Police arrested a 19-year-old driver of a second vehicle at the scene under the suspicion that he was driving under the influence. He was taken to St. Michael’s Hospital with serious injuries.
The crash happened at around 2:50 a.m., according to a news release put out by Ontario Provincial Police.
The teen was driving a GMC Envoy at the time it crashed with a Dodge Caravan, killing the 49-year-old driver and a 16-year-old young woman. Both victims were pronounced dead at the scene.
The names of the victims will not be released until their families are notified."

"BLACKMAN TWP, MI — Drivers involved in a rollover crash this morning at N. West Avenue and Boardman Road went to Allegiance Health with minor injuries.

A full-size van crashed into a GMC Jimmy, flipping the SUV on its side. The driver of the Jimmy, an 85-year-old Jackson woman, was trapped inside the SUV, witnesses said.

Witnesses told responding Blackman-Leoni Township public safety officers that the Jimmy had been north on West Avenue and attempting to make a left turn onto Boardman Avenue. The light turned red and the Jimmy continued with the turn, public safety officer Tom Brower said.

The van was west on Andrew Avenue heading through the intersection to Boardman when it crashed into the Jimmy. The crash happened around 11:30 a.m. Officers had the crash clear just after noon.

Brower said the driver of the Jimmy was found at fault in the crash. He was heading to Allegiance to interview both drivers.

Bonnie Pond, of Brooklyn saw the crash. She was waiting at the Boardman and West intersection waiting to turn onto west.

"I'm glad my light was red," Pond said.

Pond said when the pulled into the Shell station at the intersection, employees from the gas station were inside the SUV talking with the driver. Christopher Klavon heard the crash as he pulled into the gas station. He called 911 and was told several people had already reported it."

"Westbound traffic on Montauk Highway in Water Mill was backed up for more than 2 miles on Friday morning following a crash involving a car, an ice cream truck and meat delivery truck.

One of the truck drivers sustained minor injuries to his face and head and was taken to Southampton Hospital by Southampton Volunteer Ambulance.
The chain reaction accident occurred at about 9:30 a.m. on Friday and appeared to involve three vehicles each smashing into the back of each other. The cause of the crash was not immediately known. The ice cream truck suffered heavy damage to both its front and rear. The meat delivery truck, whose driver was the only person injured, sustained heavy front end damage.
Firefighters and fire police standing by in the already sweltering heat were rewarded with ice cream bars from the smashed truck."

"BOYNTON BEACH — City police have identified a man who died in an automobile accident that closed Boynton Beach Boulevard in both directions just west of the Interstate 95 overpass for nearly four hours Friday afternoon.
Police identified the man as (Censored), 79, of Boynton Beach.
The crash, which involved five vehicles and a trailer, occurred at about 2:35 p.m. Friday. Two other people involved in the crash were taken to Bethesda Memorial Hospital with minor injuries, police spokeswoman Stephanie Slater said.
Pierre was behind the wheel of a gold four-door Acura headed westbound on Boynton Beach Boulevard at a high rate of speed, city police Sgt. Rich McNevin said. When the car reached the intersection of Old Boynton Road, the traffic signal was red, but the Acura kept going and "plowed" into the back of a pick-up truck that had stopped for the red light, McNevin said.
The collision sent both vehicles into the intersection, where they hit several other vehicles, including some that were crossing the intersection to drive eastbound on Boynton Beach Boulevard, police said.
According to information released Friday night, the pickup struck a second truck. Pierre's Acura struck the trailer on another vehicle. The trailer then hit a fifth vehicle.
The occupants of that vehicle were the ones taken to the hospital with minor injuries.
City police will consider whether Pierre had a medical episode prior to the crashes.
Frank Kuzak, 64, of Boynton Beach was preparing to turn north from Boynton Beach Boulevard onto Old Boynton Road when the front left side of his car was struck. He said he thinks something had happened to the driver of the Acura because he didn't appear to make any effort to stop.
"No squealing. Nothing," Kuzak said. "All I heard is crash."
Boynton Beach Boulevard was closed westbound from Industrial Way and eastbound to the 800 block near the Casablanca Isles community while officers investigated the crash."

Child dies following two-car accident.

"CONNELLSVILLE TWP. — Two girls were killed and another person injured on Friday following a two-vehicle accident on Breakneck Road.Fayette County Coroner Phillip Reilly said that (censored), 12, of Connellsville died in the accident that occurred at 1:40 p.m. on Friday. State police said the other girl, (censored), 14, of Connellsville, was taken to UMPC/Children’s Hospital. The Allegheny County Coroner’s office said Darrell was pronounced dead at 3:50 p.m. on Saturday at the hospital. Autopsy results were not available on Saturday.

Reilly said the driver, Victor’s mother, Erica, 41, of Connellsville was taken to tertiary trauma center at UPMC/Presbyterian Hospital. A hospital spokesman said she was discharged on Saturday.

Reilly said Dr. Larry Stept pronounced (censored) dead at 3:06 p.m. on Friday in Highland Hospital. Reilly said based on reports from the accident, she died as a result of blunt force trauma with lethal head injuries and pelvic fractures. He said Victor did not suffer because she was unconscious.

State police reported that Erica Victor’s vehicle, a Dodge Spirit, lost control on the snow-covered Breakneck Road and crossed the center line into the northbound lane before it was struck in the passenger side by the second vehicle, a GMC Sierra.

The driver of the second vehicle was not injured, according to emergency personnel at the scene. State police have not identified the 66-year-old man from Connellsville. Reilly said (censored) was not ejected from the vehicle, but was trapped inside the car along with her mother and Darrell when emergency personnel arrived. Robbie Leiberger, assistant fire chief of the Connellsville Township Volunteer Fire Department, said it took nearly 20 minutes for the rescue crews to remove them from the car. The investigating officer, state police Trooper Michael Larkin, said the accident remains under investigation. Leiberger said that snow-covered roads likely contributed to the accident. “It was really nasty out there,” he said. Bullskin Township Volunteer Fire Department, Fayette EMS, state police and the Connellsville Township road maintenance crew also responded to the accident."

Police investigating fatal crash on South Shields Street.

"The woman who died in this morning's rollover in south Fort Collins is identified as (Name), 24, of Broomfield, according to Larimer County Coroner's Office.
The one-vehicle accident was discovered by a passing motorist at about 3 a.m. on South Shields Street north of Trilby Road.

The green GMC sports utility vehicle was northbound on Shields and appears to have lost control and and ran off the right side of the road. The driver appears to have then over-corrected and the SUV went off the left side of the road, rolling several times.

Garcia was ejected and died on the scene, according to Fort Collins Police Services.

She is found to have died by accident of multiple blunt-force trauma, according to the coroner's office, which performed an autopsy today.

It is unknown whether speed, drugs or alcohol contributed to the accident, and those questions will not likely be answered until toxicology tests are conducted by Larimer County Coroner's Office, according to police.

South Shields Street between West Harmony Road and West Trilby Road was closed for about seven hours during the investigation.

Anyone with information regarding the accident who has not provided police with a statement is asked to call officer David Kaes at (970) 221-6555.

This is the second fatal crash in Larimer County so far this year."

Head-on crash in Falls sends three to hospital.

"FALLS — Three people, including an 18-month-old, were rushed to a hospital Wednesday morning after two cars collided on Oxford Valley Road near Andover Road, police said. The accident occurred about 11:30 a.m.

One vehicle was traveling northbound while the other vehicle was traveling southbound, police said. The road was shut down while police investigated.
Police said the southbound vehicle, a GMC Jimmy, driven by Wendy L. Barnes, 34, of Levittown, crossed the double yellow line for an unknown reason and collided head-on into a northbound Toyota.
The Toyota was driven by Tina Conturso, 38, of Middletown, who was with her daughter, Gianna Conturso, police said.
All three were taken to St. Mary Medical Center in Middletown. Gianna, who was properly secured in a safety seat, was treated and released.
Conturso and Barnes were to remain at the hospital for observation, police said.
